Why bulk drug substances compounding is hard to verify

Three structural features of compounding oversight make verification harder than it looks, and they apply to this topic as much as any other.

Salt forms are not the same active ingredient

Some compounded products have used semaglutide salts — semaglutide sodium or semaglutide acetate — rather than semaglutide base. These are not the same active ingredient as the base form in the approved products, and FDA has stated the salt forms are not appropriate for use in compounding.

This matters for verification because it is checkable. A programme that will not state its exact salt form and concentration cannot be assessed on safety at all, and the refusal is itself the finding.
