Compounding Pharmacy License Checker

Search legal name; state; license number; status; expiration; discipline; source; last verified.

Direct answer

This tool runs entirely in your browser against the same canonical datasets that drive the visible pages and the public downloads, so the tool and the pages cannot disagree. It reports what a named public record says on the date it was checked. It issues no badge, score, ranking, or safety verdict of any kind.

Key findings

  • Primary focus: Search legal name.
  • Registration with FDA is not FDA approval, and a state licence is not a quality assessment.
  • Unverified fields render fail-closed; no badge, score, or ranking is generated from missing data.
  • Current and historical records are separated and labelled distinctly.
  • Named entities may respond to any record through the right-to-respond process.

Pharmacy licence checker

State boards of pharmacy hold licence records; FDA does not. This tool routes you to the correct register and tells you what a licence does and does not establish.

Reading a licence result
What a state licence establishesWhat it does not establish
The entity met the board's requirements at issueThat any preparation was made correctly
The licence had not lapsed when you checkedThat the most recent inspection passed
The scope of compounding permittedThat no enforcement action is pending
The licensed legal entity nameThat the trading name you searched is the same entity
Nonresident licensure. A pharmacy shipping into a state generally needs a nonresident licence from that state as well as its home-state licence. Check both. This tool reports what a named public record says on the date it was checked. It does not assess quality or safety, and it issues no badge, score, or ranking.

What a result from this tool means

What this means

  • It tells you which board holds the record and what to search for.
  • It separates resident from nonresident licensure, which are different records.
  • It gives you the exact questions that produce a checkable answer.

What this does not mean

  • That this platform has checked any specific pharmacy's licence.
  • That a current licence implies product quality or inspection outcome.
  • That one state's licence covers shipping into another.

What this tool can and cannot settle

A lookup tool answers one question well and creates a temptation to over-read the answer. This one reports what a named public record says on the date it was captured. It does not assess, score, or certify anything, and a result is a starting point for verification rather than its conclusion.

The specific limit worth understanding: a snapshot is not a live query. FDA updates its outsourcing facility register weekly and state boards update continuously, so a result here confirms what the record said on the snapshot date and nothing about today. For anything consequential, go to the source register directly — every result links to it.

Limitations of this record.
  • Results reflect a dated snapshot, not a live query against the source register.
  • FDA updates its outsourcing facility register weekly; state boards update continuously.
  • Self-reported registration information on FDA's list has not been verified by FDA.
  • Absence of a result is absence from this snapshot, not evidence that no record exists anywhere.

What to establish before paying anyone

Eight items, all answerable in writing before money changes hands, and all checkable against a public record afterwards. A programme that answers all eight has demonstrated something meaningful. A refusal on any of the first three is itself the finding.

  • Which pharmacy prepares the medication, by exact legal name — not a trade name.
  • That pharmacy's state licence number, and which board issued it.
  • Whether it is a 503A pharmacy or a 503B outsourcing facility, and for 503B claims, confirmation on the FDA register.
  • The exact active ingredient and salt form — base, not sodium or acetate.
  • The concentration in mg/mL, and the volume corresponding to your prescribed dose.
  • The beyond-use date assigned and the basis for it.
  • The named prescribing clinician and how to reach them about a side effect.
  • Whether any 'research use only' language appears anywhere in the materials.
Why these and not others. Each item resolves to a record someone else holds — a state board register, the FDA outsourcing facility list, a dispensing label. Questions that can only be answered by the seller, and never checked, are not verification.
Where the detail lives. The six parties in a compounded prescription, and which register holds each, are set out on the provider–pharmacy relationships page.
